Polycystic kidney disease (PKD) is an inherited disorder in which clusters of
noncancerous, fluid-filled sacs (cysts) develop within the kidneys.The disease
can be inherited in autosomal dominant and recessive forms.Both autosomal
dominant and recessive polycystic kidney disease are conditions with severe
associated morbidity and mortality.If you are diagnosed with autosomal dominant
polycystic kidney disease (ADPKD), your GP will probably refer you to a renal
consultant or a nephrologist (doctor who specialises in treating kidney
conditions).
Recent advances in the understanding of the genetic and molecular
pathogenesis of both ADPKD and ARPKD have resulted in new, targeted therapies
designed to disrupt cell signaling pathways responsible for the abnormal cell
proliferation, dedifferentiation, apoptosis, and fluid secretion characteristic
of the disease:
High blood pressure:Controlling high blood pressure may delay the progression
of the disease and slow further kidney damage.Two medications widely used to
lower blood pressure in people with ADPKD are angiotensin-2 receptor blockers
(ARBs) and angiotensin-converting enzyme (ACE) inhibitors
Complications of cysts:Rarely, when kidney cysts are causing severe pain or
obstructing other organs or blood vessels, you may need to undergo surgery to
drain the cysts.
Pain:In many cases of ADPKD, treating the underlying cause of pain, such as
kidney stones or a UTI, will help relieve the painful symptoms.Often, the pain
is mild and you can control it with over-the-counter medications containing
acetaminophen.If you have symptoms of chronic (long-term) pain, you may be
referred to a specialist pain clinic.Possible treatment options the clinic may
provide include amitriptyline.
Bladder or kidney infections:Bladder or kidney infections. Prompt treatment
of infections with antibiotics is necessary to prevent kidney damage.
Kidney failure:Kidney failure. If your kidneys lose their ability to remove
wastes and extra fluids from your blood.There are currently two effective
treatments that can be used on a long-term basis:
dialysis – a machine replicates the functions of your kidneys
kidney transplant – a kidney is removed from a donor and implanted into a
patient
