The meta-analysis, by Wisit Cheungpasitporn, MD, of Mayo Clinic in Rochester, Minn., and colleagues, included 7 studies. A history of kidney stones was associated with a significant 76% increased risk of RCC and a 2.1 times increased risk of TCC.

Extended Hemodialysis May Improve Phosphate Control

Since the 1960s, when hemodialysis first became a practical treatment for kidney failure, we've learned much about how to make hemodialysis treatments more effective and minimize side effects.  Hemodialysis can be an outpatient or inpatient therapy. Extended treatment time (TT) on hemodialysis (HD) may improve serum phosphate control, according to study findings presented at the 2014 Kidney Week meeting.

Investigators at Imperial College Hospital NHS Trust and North Bristol NHS Trust in the U.K. compared extended TT (6 hours) and standard TT (4 hours) on HD in a randomized, crossover study that included 29 patients. During the extended TT phase of the study, serum phosphate decreased significantly from 1.67 g/dL before treatment to 1.27 g/dL after 6 hours of treatment.

During the standard TT phase, serum phosphate increased significantly from 1.51 g/dL before treatment to 1.94 g/dL after 4 hours of treatment.

In addition, during the standard TT phase, researchers observed a significant increase in extracellular water, whereas during the extended TT phase, patients had no significant change in hydration status.

In a statement prepared by the National Kidney Foundation (NKF), which publishes the American Journal of Kidney Diseases, corresponding author Nicole Lurie, MD, Department of Health and Human Services Assistant Secretary for Preparedness and Response, stated that the research clearly showed that delaying dialysis can have devastating health effects for patients with ESRD.

“The good news is that we saw a lot of patients receiving dialysis before the storm hit. That type of advance planning by patients and their facilities should become routine nationwide,” she said. “Everyone involved should know what to do when their facilities might close—patients should know where to go, and facilities should be able to provide a surge in early dialysis care so treatment is not delayed. At the end of the day, that helps people and their communities be more resilient.”

Room for improvement
Dr. Lurie pointed out that 40% of patients did not receive early dialysis, meaning “there is still plenty of room for dialysis patient and facility improvement. I hope these findings serve as a rallying cry not just for the dialysis community, but for all people with any type of chronic health condition and their care providers to plan for emergencies.”

“This is an important analysis of the impact of Hurricane Sandy in October 2012 on individuals treated with in-center hemodialysis that ultimately helps the dialysis community better anticipate what might go wrong, improve emergency plans for disaster scenarios, and more rapidly respond to protect the health of our patients during emergencies,” said Joseph Vassalotti, MD, NKF's Chief Medical Officer. “The study reflects another step forward to improve the education and training of patients and dialysis clinic staff, following the widely recognized poor response to Hurricane Katrina in August 2005.”

While the condition, called hyponatremia, is rare, it happens. Long-distance runners are at highest risk for acute hyponatremia (meaning the imbalance happens in less than 48 hours), but anyone can get in trouble if they drink water to excess without replacing essential electrolytes and minerals. Extreme overconsumption of water can also strain the kidneys and, if drunk with meals, interfere with proper digestion.

Chronic hyponatremia, meaning sodium levels gradually taper off over days or weeks, is less dangerous because the brain can gradually adjust to the deficit, but the condition should still be treated by a doctor. Chronic hyponatremia is often seen in adults with illnesses that leach sodium from the body, such as kidney disease and congestive heart failure. But even a bad case of diarrhea, especially in children, can set the stage for hyponatremia. Be on the lookout for symptoms such as headache, confusion, lethargy and appetite loss.

That said, there is no scientific evidence to back up the very specific and well-worn advice that you need to drink eight, 8-ounce glasses of water a day (a.k.a. the 8 x 8 rule). In 2002, Heinz Valtin, MD, a retired physiology professor from Dartmouth Medical School and author of two textbooks on kidney function, published the definitive paper on the subject in the American Journal of Physiology. He spent 10 months searching medical literature for scientific evidence of the 8 x 8 rule only to come up empty-handed.
